program crashes

Trying to run TSWIN. It is a program that has a database of members and tracks activities.
Installed and was working fine until i tried to add a new member. Each time I click 'ok' after entering data
it crashes.

where do i start looking for the culprit?

The most useful thing would be a log file without and flags that had the crash almost immediately. Instructions to gather such a log file are available here:
