The only way (I think) this can be done is with non-apple software support of a non-Apple Keyboard.
I have a Logitech keyboard which is recognized by Apple "preferences/keyboard" and I cannot change anything useful there. (Apple apparently does not differentiate between L & R cmd/ctl/alt/shit. even though they are different keycodes.)
However, I have a Kensington Trackball which uses "Trackball Works" (formerly Mouse Works) from Kensington, and it allows me to change all four buttons and 2 cords to be many different things.
Other than that, most games allow re-defining keystrokes "in-game."
