Programs->Run Command... , select The Fallen-bottle and as command choose the patch you just downloaded. Install the patch by pressing Run.
If the programm doesn't start or the screen remains black you need to find a way to circumvent the CD protection.
When you reach the DS9 First-Time Configuration choose the "Direct3D Support" Device. This works best. OpenGL Device works also (it's in the unsupported section).
Changing resolution doesn't work very well. Sometimes it works. Sometimes when you change the resolution, the screen is messed up or stays white. Restarting the program might fix this. Some resolutions don't work well in the game, e.g. 1400x900 works after some restarts, but part of the GUI is outside of the screen. 1024x768 works best.
Apart from the resolution change problem, the game works very well.
