Here are the installation steps for the GOG.com version:
Install the game as an unsupported application into a new WinXP bottle called Sanitarium. Use the GOG.com installer (setup_sanitarium.exe) and in the fist setup dialog click Options and uncheck the Adobe Reader checkbox.
Now comes the tricky part! The game won't launch if you try to start it normally from the CrossOver Programs menu or by clicking it's shortcut. Follow the steps below to create a new shortcut that will work:
Select the Programs > Run command menu in CrossOver.
Select the Sanitarium bottle in the Use bottle combo box at the top of the dialog.
Press the Browse button and select the file sntrm.exe in the Program Files/GOG.com/Sanitarium directory inside the game bottle. The full path name should resemble something like this:
Press the Save Command to Programs Menu button. A Finder window will appear shortly containing a shortcut called sntrm that you have just created. Rename this to something sensible (like Sanitarium) then select the Configure / Refresh Programs Menu menu in Crossover.
When starting the game using the newly created shortcut, a pop-up window will appear with the message:
Operation under Windows NT is not supported. Continue game?
Press Yes and the game will start in fullscreen mode.
