Microsoft Word integration may not install the first time, so if you can't get the toolbar to show up, simply reinstall Endnote, choose custom install when prompted, and make sure the "Word Templates/Add-ins" is checked. This should cause the "Cite while you write" plugin to register with Word.
This has essentially been fixed in Crossover Office 5.0, at least with Fedora 3.
