Start by making a Win98 bottle. (Other bottles may work now, but in the past they produced errors.)
You will then want to install the following:
Core Fonts
Internet Explorer 6
MDAC - 2.5
Windows Installer 2.0
.NET Framework 2.0 For Win98 Bottles (The license may be jiberish, just agree. Although it may say the installation failed at the end, it did what it needs to do.)
ClickFORMS should then install using the Unsupported Software procedure.
