The GOG installer for Carmageddon is a pre-packaged DOSBox app, which in itself isn't a bad thing. The settings that come default are mostly OK, except the issue is the FPS it plays with as default are barely playable.
To fix this, open the DOSBox .conf files, there are multiple as the GOG Installer comes with Splat Pack as well. In the .conf files, the thing to look for is 'cycles'. Update cycles to something outlandish, as 'max' doesn't get the job done. For this to be playable, I set this to 90000.
When launching, do not use 3DFX as this doesn't seem to work well no matter what the settings are. Use the regular exe via the packaged DOSBox exe. Launching the main Carmageddon exe will give you a DOS Prompt to choose which one of 3 options, choose the option without 3DFX.
After updating cycles, the game plays with very much acceptable FPS.
