I just tested MathType 5.2 under CX4.0 and there are a couple of major regressions. First, the install went fine; no errors. Now for the problems. If Word XP is started first and then an equation is inserted by pressing a button on the MathType toolbar, Word will crash and restart. Once restarted, a MathType equation can be inserted without Word crashing. However, it gives an error that the equation preferences could not be passed to MathType. Also, the Word crash can be circumvented by launching MathType prior to Word.
The other main bug is when creating equations in MathType. If a symbol is selected from one of the drop-down boxes, the MathType window will become unresponsive (none of the menus or toolbars wil work). Normal functionality can be restored by simply minimizing the window and then restoring it. After performing the minimize-restore sequence once, any number of menu/toolbat selections can be made.
As a result of these regressions, I am lowering the support level to bronze.
