I noticed that in the screen shots, Fallout was running in a windowed mode on Mac OSx. Does any one know how this was done? I can exit fullscreen mode, but then the game is stuck in the top left corner of the screen.

Did you try to exit and restart the game after changing the settings? Unlike Windows, MacOS can't switch the title bar on and off on an existing window; That causes problems for Windows apps which try to switch between fullscreen and windowed mode without creating a new window.
