What is your system specs? I have been googling about this the last few days, and it appears that it is slow because of sloppy console port... It seems to run well on CPUs with at least four cores, but not-so-well on dual cores... That is, someone said that the game uses CPU to process stuff that should be done on GPU...
In any case, check out this:

After following those config tips (should work for both multiplayer and single player, edit config_mp.cfg and config.cfg respectively) the game is playable on most areas, especially indoors. Some outdoor areas are still a problem though. Screen resolution appears to have no effect here. Setting textures to low seems not to improve anything, so I have set it to extra. I have a 3GHz core 2 duo, nvidia 260gtx.
For cxgames specific tips: you might get some boost by setting UseGLSL to disabled with regedit, at least some people in winehq claimed so. I'm not sure what the default value is, though...
Same problem here, someone has already solved?
Greetings
